Laboratory Inspection Form

It is the aim of the Chemical Hygiene Committee (CHC) and Environmental Health & Safety (EHS) to work cooperatively with Principal Investigators and laboratory workers to achieve compliance with University safety policies, the Chemical Hygiene Plan and governmental regulations. Unsatisfactory items and laboratory safety violations will be handled, following the steps outlined in the Chemical Hygiene Compliance Policy (https://www.udel.edu/EHS/CHCCompliancePolicy.pdf). Issues that represent an immediate or imminent hazard to University Personnel, risk to the environment or potential to cause damage to University facilities are classified at Category 1 Deficiencies and must be rectified immediately.

Researchers who are interested in submitting an application to any of the limited submissions should first review the announcement carefully to ensure eligibility. All internal limited submission white papers must follow the template found here. Applications not adhering to this requirement will be returned without review. Material Transfer Agreement Required Webform

The Webform is the first step in requesting a Material Transfer Agreement (MTA) and is required by the Research Office prior to approving any request.  Please download and complete the UD Webform, including the MTA attachment, as completely and accurately as possible and submit.  The system will route the completed form to the appropriate approvers in the Research Office or the requestor’s Department.  The Research Office will notify the requestor when the agreement has been fully-executed and it has been approved to ship materials.  Templates for the MTA attachment are provided below.

NIH Salary Cap Calculator

In order to strengthen compliance with the National Institutes of Health (NIH) Salary Cap, the Research Office has developed a tool to clarify and simplify the salary cap calculation. This tool is intended for use by both department administrators and the Research Office Post Award team to monitor cap compliance during the life of the project and close out at the end of the project. Final determination of salary JVs to move direct salary to cost share in the case of cap overage will be made by the Research Office using the calculator in coordination with the department. Outdoor C-14 Incubation Record and Checklist

The proper completion and submission of this worksheet is required by the Radiation Safety Committee for each incubation using radioactive materials on the dock outside the Marine Operations Building. Failure to do so may result in the issuance of a permit violation.

Permit Supervisor Training Certification for Radiation Material Users

General form to record degree of training for personnel to be used for department records.

Permit Supervisor Training Certification for X-Ray Workers

Certification for worker qualifications to use x-ray machines for departmental records.

PI Eligibility Approval Form

Principal Investigators and Co-Principal Investigators have primary institutional responsibility for providing scientific/technical leadership and administrative and financial management of sponsored projects. The guidelines for Principal Investigator Eligibility require this form to be submitted as a request to serve as Principal Investigator (PI) on a project. The PI will be required to provide a CV and proper training and experience documentation.

Proposal Development Assistance Request Form

The Research Development Office (RDO) serves as the support office to help faculty navigate today’s challenging research funding landscape and prepare competitive grant proposals. RDO provides support and management for large and/or multi-department proposals, where RDO serves as a point of contact for PI for all aspects of a grant application, including proposal project management, finding the right person(s) and UD units to leverage, grant writing and graphics, letters of support from UD leadership, equipment and other cost shares, and special exceptions – all leading to submitting a successful proposal. Use this form to request aid in the proposal preparation process.

Proposal Review Panel Interest Form

A variety of sponsoring agencies offer research, career, scholar or educational funding opportunities that limit the number of applications that can be submitted. An internal selection process has been established by the Research Development Office to assist with reviewing these opportunities and is always searching for new reviewers whose expertise is as close as possible to the content area. If you are interested in serving as a reviewer, please provide your name, college, department up to three areas of expertise to be added to our list of reviewers by filling out this form.
