Research Subaward Agreement: Attachment 3B (A two page pdf)

Research Subaward Agreement that is needed for FFATA reporting. It is important to have both pages filled out. Page 1 is information needed for subrecipient contacts including UEI details, administration personnel information and location. Page 2 details includes the names and compensation of the five most highly compensated officers.

Residual Balance Transfer Request Form

Use this form if a residual funding balance remains on a fixed-price sponsored program after all allocable direct and F&A costs have been charged, the PI may use this form to request the residual amount be transferred to a non-sponsored departmental account per the Residual Balance Policy. Residual balances will be distributed proportionately between direct costs and F&A costs, with the direct cost amount being transferred to the administering department. Email completed forms to your Sponsored Research Accountant for final Research Office approvals and processing.

Sponsored Research Program Income Procedure & Routing Form

Subrecipient Commitment Form

All non-FDP (Federal Demonstration Partnership) sub recipients must submit this form when submitting a proposal to the University of Delaware (UD). It provides a checklist of documents and certifications required by sponsors, as well as an area for the authorized official to sign.

Subrecipient Letter of Intent

The Subrecipient Letter of Intent form is referenced in the Proposal Checklist for proposals involving subawards.

UD Individual Development Plan Assessment Form (IDP Form)

University of Delaware IDP Assessment Form
The University of Delaware requires Individual Development Plans (IDPs) to be completed for graduate students and postdocs funded by NIH (regardless of funding mechanism) each year. The use of IDPs for all graduate students and postdocs is encouraged as a means for professional development and career planning. Additionally, other funding agencies are beginning to request IDPs. To aide in this requirement the UD Research Office is providing an IDP Assessment Form that will help with this effort.
